Time of Clubs: 5-45pm to 7:30pm
Registration Costs: $30 per clubber; $50 per family(includes vests & books)
Clubs offered: Puggles, Cubbies, Sparks, TNT, TREK for teens

Who will pick up the child at the end of each AWANA day?
Parent or Legal Guardian Consent
By my submitting this form, I indicate the following:
I am the parent or legal guardian of this child.
I give permission for my child to participate in all activities at Landmark Church’s AWANA Club each Sunday, beginning Sep 7, 2025, unless otherwise noted.
I authorize Landmark Church to get medical attention for my child in the event of an emergency.
I give permission for my child to be photographed or videoed during AWANA activities, and I allow pictures and/or videos to be used in any Landmark Church publications, advertisements,or social media.
